At times, you may need round the clock care that can be provided on the In Patient Unit.

Do you find it difficult to have conversations with those in your care about end of life? People have the right to make choices in their care including end of life medical decisions. However, these conversations can be challenging for many.
Advance Care Planning (ACP) is an important component of the Gold Standards Framework. Attending this evidence-based workshop will support you to build confidence in having these sensitive conversations. You will learn about key components and benefits of having ACP discussions with people in their last years of life.
This interactive full day workshop will include the use and guidance on ACP in different settings, ACP for people with dementia, discussion of common challenges, solutions and communication skills.
The session is led and facilitated by Gail Edwards, Nurse Champion, with input from Judy le Marquand from the Education team and the Emotional Support Team.

This course is suitable for all healthcare workers at all levels interested in learning more about having advance care planning discussions.
