Referrals
All referrals must be made with the consent of the patient and agreement from their GP.
Who can make a referral?
- Any health and social care professional (for example GPs, social workers, district nurses) who knows the patient.

Our Education Team, supported by specialist clinical staff, provides an innovative planned programme of education, underpinned by the most current evidence in end of life care. All of our courses are run by expert clinicians who have been trained to teach in the subjects they deliver.
